- New Club Packet
For clubs that are new on campus or for clubs that are restarting after being inactive for more than four semesters. - Continuing Club Packet

A "fill in the blank," customizable club constitution. - Additional Club Advisor Form
Use this form to designate more than one CSM faculty or staff member as an advisor to your club. - New Club/Trust Account Authorization Form
Use this form to establish a new account for your club; after completing this form, it must be submitted to the Center for Student Life for processing. - Club/Trust Account Authorized Signatures Form
This form must be submitted to the Center for Student Life each semester with the authorized signatures for your club's account.
- ASCSM ICC/Student Senate Proposal Form
Use this form when requesting funding from the Inter Club Council and the Student Senate; please check with the Office of Student Life for when proposals are due.
- Activities Request Form
This form is required for any events/activities that will take place on or off the College of San Mateo campus that involve student clubs or organizations. - Liability Release, Behavior Standards, and Medical Consent Form
Each participant in an off-campus event sponsored by an official CSM club or organization must complete this form. - Fundraising Application
Application to conduct a fundraiser on or off campus.
