Purpose of the Form

This form should be used to report student behaviors of concern, including but not limited to signs of distress, disruptive behavior, or changes that suggest a student may need support. If there is an immediate safety concern or a life-threatening emergency, call 9-1-1 first and contact Public Safety at (650) 738-7000 when it is safe to do so.

What happens to this Referral

This report will be forwarded to the VPSS Office and will be reviewed by the CARES Team. You, the reporter, will not be revealed to the student of concern. All reports submitted through this form are monitored Monday-Friday, 8 am - 4:30 pm.
