Getting Started with the Disability Resource Center

1. Complete ALL of the following forms. Not completing the application for services (providing the verfication of disabilty) and the consent form will result in a delay of obtaining an appointment:
   • DRC Application for Services
   • Consent for Release of Information
   • DRC Voter Preference Form
2. Upload supporting disability documentation (for example, a doctor’s report or psycho-educational report with IEP) with the application.
3. Schedule an appointment with a DRC Counselor to:
   
   • Create a student educational plan (SEP) of classes,
   • Discuss appropriate academic adjustments, auxiliary aids, and services based on an interactive process and your educational needs,
   • Review your rights and responsibilities as a DRC student at CSM.
4. If relevant, schedule a follow-up appointment(s) for alternate media or assistive technology needs.

Remember, once you are established with DRC, be sure to request your academic accommodation letter every semester.
