Getting Started with the Disability Resource Center

1. Complete the following forms:
   • DRC Application for Services
   • Consent for Release of Information
   • DRC Voter Preference Form
2. Bring in/upload supporting disability documentation (for example, a doctor’s report, psycho-educational report with IEP) to the DRC office.
3. Schedule an appointment with the DRC Director to:
   
   • Complete the intake process,
   • Arrange for appropriate academic adjustments, auxiliary aids, and services based on an interactive process and your educational needs,
   • Discuss your rights and responsibilities as a DRC student at CSM.
4. Schedule follow up appointment(s) for class selection - Student Educational Plan (S.E.P.) and/or assistive technology.

Remember, once you are established with DRC, be sure to request your services every semester.
