Apply for Admission
New students can apply for admission in one of three ways:
- Complete the online application through WebSMART.
- Obtain a copy of the application found in the Schedule of Classes (available in Administration Bldg 1, Second Floor).
- Print a copy of the application form (PDF).
Submit your application form either in person at the Office of Admissions and Records
(Bldg. 1, Second Floor), by fax (650 574-6506) or by mail to:

Register for Classes
After submitting the application form, you will receive an appointment date by mail so you can register for classes using WebSMART online or by telephone registration.

Adapted P.E. Information Card
To facilitate the registration process, students are required to fill out an Adapted PE Information Card (PDF). This information will be used to help us fine tune your exercise program as well as complete the Medical Verification form that will need to be completed by your physician.
